Intensive Outpatient Program (IOP)
An Intensive Outpatient Program (IOP) provides structured, multi-day addiction treatment while participants live at home. Typically running 3 to 5 days per week for 3 to 4 hours per session over 8 to 12 weeks, IOP bridges the gap between weekly counselling and Residential care. It is suited for individuals with moderate addiction, supportive home environments, and obligations — work, family, school — that make residential treatment impractical.
How IOP Differs from Standard Outpatient
Standard outpatient runs 1 to 2 sessions per week and works well for mild addiction or the maintenance phase. IOP runs 3 to 5 sessions per week — more contact, more structure, more accountability. Research shows that IOP produces better outcomes than weekly counselling alone for moderate to severe addiction.
Who IOP Is Best Suited For
- Moderate addiction not requiring 24-hour medical supervision
- Stable, supportive home environment without active substance use
- Employment, school, or caregiving responsibilities
- Stepping down from residential treatment or medical detox
- Previously tried and relapsed with standard weekly outpatient
IOP is not appropriate for severe addiction requiring medical supervision, those whose home environment includes active substance use, those with suicidal or psychiatric crisis requiring hospitalization, or those unable to commit to the 3 to 5 day per week schedule.
Core Components of IOP
Group Therapy
The foundation of IOP. Facilitated peer groups of 6 to 12 participants. Common themes: triggers and cravings, relationships and communication, grief and loss, anger management, healthy coping, and relapse prevention.
Individual Therapy
One-on-one sessions 1 to 2 times per week. CBT, Motivational Interviewing, trauma-focused work, and personalized relapse prevention planning.
Relapse Prevention
Identifying high-risk situations and developing specific coping strategies. Creating a crisis plan. Building a sober support network for high-risk moments.
Family Sessions
Many IOPs include family therapy or education components addressing enabling behaviour, communication breakdowns, trust repair, and how to support a loved one in recovery.
Drug Testing and Medication Management
Regular urine drug screens maintain accountability and provide early warning of relapse. Coordination with prescribers for buprenorphine, naltrexone, or other MAT medications.
Cost and Access in Canada
Public IOP is available through provincial health authorities and community addiction services at no cost. Wait times typically 1 to 4 weeks.
Private IOP costs $5,000 to $15,000 for a full 8 to 12-week program. Often accessible within 1 to 7 days. Private insurance, EAP benefits, and HSA accounts may cover significant portions.
After IOP: Continuing Care
Completing IOP is a milestone, not a finish line. A strong transition plan includes stepping down to standard outpatient counselling (weekly sessions for 6 to 12 months), attending mutual aid groups 3 to 7 times per week in early post-IOP recovery, sober living housing if the home environment remains high risk, and continuing care check-ins with your primary counsellor.
